I understand that there may be a risk in the learning and practice of Taekwondo that it is possible to cause injuries. In joining Maiwand Taekwondo, I, as a result of this, absolve Maiwand Taekwondo, the instructors, and all students of Maiwand Taekwondo from any liability for any loss or injury that I may sustain while practicing, being taught, or competing in Taekwondo. I clearly understand that my participation in Taekwondo is entirely at my own risk. I will further abide by the rules and regulations of Maiwand Taekwondo if I am accepted as a trainee.
I, with this guarantee, do not have a criminal record, and I, at this moment, confirm that I don't suffer from any illness, disease, or any other mental or physical disorder that can stop me from practicing Taekwondo.
I agree that I or my child can be photographed and videoed during the training and competition.

NOTE: £ 60 is the membership fee, and the annual insurance fee is not refundable.

The monthly fee is £25 per month for under 14 years and £30 for 14 and above and should be paid in advance in the first week of the month and is not refundable either. This agreement may be terminated at any time with one month's notice in advance.
